 SPRAVATO, also known as esketamine, is an FDA-approved prescription nasal spray for adults with treatment-resistant depression. It may be used by itself or together with an oral antidepressant. 

SPRAVATO works differently from traditional antidepressants. It is believed to affect the brain’s glutamate system, although the exact way it improves depression symptoms is not fully understood. For some patients, this different treatment pathway may provide meaningful relief when previous antidepressants have not worked well enough.

You may be a candidate for SPRAVATO if you are an adult who:

Eligibility is based on more than the number of medications you have tried. During your consultation, we will carefully review your symptoms, medical and psychiatric history, previous treatments, current medications, and treatment goals. We will also discuss possible risks and determine whether SPRAVATO can be used safely as part of your care.

 SPRAVATO is self-administered as a nasal spray while you are at our office under the supervision of our clinical team.  Because the medication may temporarily cause sleepiness, changes in perception, dizziness, nausea, increased blood pressure, or a sense of disconnection, you will remain with us for at least two hours after receiving your dose. Your blood pressure and overall response will be monitored throughout the visit. 

We have created a peaceful treatment space where you can settle into a comfortable chair, dim the lights, listen to music, rest, or simply relax while our team remains nearby.  You will never be expected to navigate the experience alone.

You must arrange for a trusted adult, rideshare service, or other transportation to take you home after each appointment. You should not drive or operate machinery until the following day after a restful night’s sleep.

 For treatment-resistant depression, SPRAVATO typically begins with an induction phase in which treatments are given twice a week for four weeks. Treatment frequency then usually decreases to once a week, followed by weekly or every-other-week maintenance treatment based on your symptoms and response.

Your schedule will be personalized to your needs. Throughout treatment, we will monitor your progress and work with you to determine the frequency that provides the greatest benefit.

 How quickly does SPRAVATO work?

Some patients may notice improvement earlier than they have with traditional antidepressants, while others require several treatments before noticing a meaningful change. Every person responds differently, and improvement cannot be guaranteed. We will monitor your symptoms throughout treatment and discuss whether it is helping.

Is SPRAVATO the same as ketamine?

SPRAVATO contains esketamine, which is related to ketamine but is not the same medication. SPRAVATO is an FDA-approved nasal spray with specific dosing, monitoring, and REMS requirements.

Will I be asleep during treatment?

Most patients remain awake. You may feel sleepy, relaxed, dizzy, disconnected, or temporarily different from your usual self. These effects generally lessen during the monitoring period, but experiences vary.

Can I drive myself home?

No. You cannot drive after receiving SPRAVATO. You will need transportation home and should not drive again until the following day after a restful night’s sleep.

Does insurance cover SPRAVATO?

Many commercial insurance plans and Medicare plans may cover SPRAVATO for eligible patients, although prior authorization and documentation of previous antidepressant trials are commonly required. Coverage and out-of-pocket costs vary by plan.

Do I need to change psychiatric providers?

No, we may provide SPRAVATO while you continue routine psychiatric care with your existing provider. With your permission, we can communicate with that provider to support coordinated treatment.
